Health Assessment

As part of our focus on illness prevention, we are here to help you improve your knowledge and understanding of the risk factors that may contribute to your developing a chronic medical condition. No matter what your age, we encourage you as our patient to take advantage of the health assessments we provide for various age groups. These health assessments provide us with a baseline from which you and we can work together to minimise your risk of health problems. 

Health assessments also permit the needs of specific groups (Aboriginal and Torres Strait Islander people, refugees and aged care residents) to be addressed in a targeted and culturally appropriate manner. All our centres provides specific health assessment services for the following groups of people : 

  • 75 years or older 

  • 45 - 49 years old individuals 

  • 40 - 49 years old individuals with risk of having diabetes 

  • 4 years old heatlhy kids check 

  • Aboriginal and Torres Strait Islander people 

 

The schedule below is recommended by the Royal Australian College of General Practitioners. Please feel free to discuss this with your doctor so that we tailor it to your particular requirements.
