The goal of the medications review is to maximise an individual patient’s benefit from their medication regimen, and prevent medication-related problems through a team approach, involving the patient’s Family doctor and the community pharmacist. It may also involve other relevant members of the health care team, such as nurses in the practice or carers. 

The clinical teams have accredited Community Pharmacists to provide this important service for our patients. The medications review process utilises the specific knowledge and expertise of each of the health care professionals involved. In collaboration with the family doctor, the pharmacist comprehensively reviews the patient’s medication regimen in a home visit. After discussion of the pharmacist’s report and findings, the doctor will work with patient together to develop a new plan of medications intake. The patient is central to the development and implementation of this plan with the doctors.
